We are committed to protecting privacy through compliance with this Privacy Policy (“Privacy Policy”).

This Privacy Policy describes how we collect, use, disclose, and otherwise process Personal Information and your related interactions with us in connection with your use of our Platform. This Privacy Policy does not apply to information we collect through other sources.

Consent to Privacy Policy

Please read this Privacy Policy carefully to understand our policies and practices regarding your Personal Information and how we will treat it, including the collection, use, and disclosure of your Personal Information. By accessing or using our Platform, you agree to this Privacy Policy. This Privacy Policy may change from time to time. Your continued use of our Platform after we make changes is deemed to be acceptance of those changes, so please check the Privacy Policy periodically for updates.

Information We Collect From You

The information we collect from you, includes, but is not limited to: name, age, gender, physical address, email address, phone number, household information, income, marital status, and personal health information such as medical history. Where you provide a mobile phone number and affirmatively opt in to receive text (SMS) messages from us, we also collect your mobile number, your consent record (including the date, time, and method by which consent was obtained), and information regarding the messages we send and your responses for purposes of providing, securing, and improving the SMS program.

How We Use Your Information

We use your personal information to match your request for insurance products and services with our network of providers of insurance, our strategic partners, products, and services.

We may also share your information with third-party contractors who provide services to us so that we can provide you with the site and its various services. These third-party contractors are prohibited from using the information we provide for purposes other than performing services for us. We may transfer your information to third parties involved in a corporate merger or acquisition with us.

We may also disclose your information to third parties when we reasonably believe we are obligated to do so by law. We may also do so to investigate, prevent, or take action regarding suspected or actual prohibited activities, including, without limitation, fraud, situations involving potential threats to the site or the physical safety of any person, and violations of any applicable law.

No Sale or Sharing of Mobile Information and SMS Consent. No mobile information, including mobile phone numbers and SMS consent (opt-in) data, will be shared with or sold to third parties or affiliates for marketing or promotional purposes. Information that you provide as part of our SMS program — including your mobile number and consent — is used solely by Champion Health and the service providers (such as our messaging platform and SMS aggregator) that we engage to deliver, secure, and support the SMS program. These service providers are contractually bound to use such information only to provide services to us and may not use it for their own marketing purposes. This restriction on sharing applies even where your information is otherwise covered by the disclosures described above in this section. We do not share mobile opt-in data with third parties or affiliates for marketing or promotional purposes under any circumstance.

SMS / Text Messaging Program

Program description. Champion Health offers a text (SMS) messaging program to members, prospective members, and other individuals who have provided their mobile number and consent. Through this program we send account and enrollment notifications (for example: welcome and verification messages, account or coverage status updates, identity confirmation, and password or security alerts) and customer support communications (including two-way conversational messages between you and our support team in response to a question, request, or open ticket).

How you opt in. You may opt in to receive SMS messages from Champion Health by (i) submitting a form on champplan.com that includes an SMS consent checkbox or disclosure, (ii) providing your mobile number and consent during employer-sponsored benefits enrollment, (iii) opting in within the Champion Health mobile or web application during account setup, or (iv) providing verbal consent to a Champion Health representative during a phone call, which we will document. By opting in, you agree to receive recurring SMS messages from Champion Health at the mobile number you provided. Consent is not required as a condition of purchasing any goods or services.

Message frequency. Message frequency varies based on your account activity and interactions with our support team. You should generally expect up to approximately ten (10) messages per month, though actual volume may be higher or lower depending on activity.

Message and data rates. Message and data rates may apply. Charges are billed by and payable to your mobile service provider. Please consult your mobile carrier’s pricing plan to determine the charges for sending and receiving text messages. Champion Health is not responsible for any charges incurred by you in connection with SMS messages.

Opting out. You can cancel the SMS service at any time by replying STOP to any message you receive from us. After you send STOP, we will send you a confirmation message and will not send you any additional messages from that program unless you opt in again. If you want to rejoin, sign up as you did the first time and we will start sending messages to you again.

Carrier disclaimer. Carriers (including, but not limited to, AT&T, T-Mobile, Verizon, Sprint, Boost, U.S. Cellular, MetroPCS, and Cricket) are not liable for delayed or undelivered messages. Delivery of messages is subject to effective transmission from your wireless service provider/network operator and is outside of Champion Health’s control.

Eligibility. The SMS program is available only to individuals 18 years of age or older with a mobile phone number issued by a participating U.S. wireless carrier. You represent that you are the subscriber of, or are authorized to provide consent for, the mobile number you submit.

Privacy of SMS data. Information you provide as part of the SMS program is handled in accordance with this Privacy Policy. As stated above, mobile phone numbers and SMS opt-in / consent data are not shared with or sold to third parties or affiliates for marketing or promotional purposes.

How We Keep Your Information Secure

We take your personal information seriously and take every effort to ensure it is protected and secure. All client data is encrypted while in transit and at rest and is maintained indefinitely, unless the client explicitly requests that its data be removed. All transmissions of electronic information containing personal information is encrypted by default via secure transmission protocols (e.g., HTTPS, SFTP).

Retention and Destruction of Group or Member (Customer) Confidential Information

Champion Health retains and safeguards all confidential customer information indefinitely. If a Customer requests that we destroy or delete their confidential information, we will do so immediately, deleting all such data from our records and files. If you opt out of our SMS program by replying STOP, we will retain a minimum record of your opt-out (your mobile number and the fact that you have revoked SMS consent) solely to honor your opt-out request and prevent further messaging, consistent with applicable law.

Cookies

We may use cookies, web beacons, and other tracking technologies to collect information about you and your interaction with the site. We may also use tracking technologies for interest-based advertising, as described below. Typically, our cookies will expire after 30 days. Most web browsers are set to accept cookies by default, but you can usually change your settings to reject cookies. Information Relating to Children

Only individuals 18 years of age or older have permission to use the site. If you are not at least 18 years old, do not use the site. Any individual who uses the site or provides their information to us represents that they are 18 years of age or older.

Changes to this Privacy Policy

We may make changes to this Privacy Policy at any time, for any or no reason, with or without notice to you. Such changes may take place due to a change or expansion in our services or policies. As you use the site, you are required to periodically check this Privacy Policy for any changes. To assist you, we provide the Privacy Policy’s effective date at the top of this page. You are responsible for reading and understanding this Privacy Policy, and your continued use of the site or our services following a change or update to this Privacy Policy constitutes your continued agreement and consent to the practices described herein.
