What is the CHAMP Plan™?
The CHAMP Plan™ is a self-funded, self-insured limited schedule for benefits plan that allows employees to receive cost-effective health benefits. Employers can customize their plans to meet the needs of their workforce rather than purchasing a policy that isn’t tailored to their specific needs. This offers more control to the employer while giving the employees access to benefits they actually value.
What Virtual Direct Primary Care Benefits are Available?
Through the CHAMP Plan™, plan members can access the full scope of virtual direct primary care services through virtual office visits. A majority of health conditions can be observed and treated virtually, so do not worry about plan members not being able to get quality care. The services available through Virtual Direct Primary Care benefits range from basic annual physicals to chronic care management.
Patients may also receive referrals for laboratory services or prescriptions for medications as needed. Other commonly used services include:
- Medical wellness sessions
- Referrals for rehabilitation services
- Referrals for specialty services
- Preventive health services
- Acute condition management

Which Conditions Cannot Be Treated Virtually?
Several health conditions are not eligible for virtual treatment. These include:
- Neurologic symptoms suggestive of stroke
- Chest pain
- Moderate to severe shortness of breath
- Trauma especially involving the head, neck, trunk, and serious trauma involving the extremities, such as amputations or crush injuries
