  • $0 & $1 copay for prescription drugs listed on our formulary (dosage and frequency limitations listed on Drug Formulary) for you & registered family
  • $0 copay for Unlimited Virtual Direct Primary Care 24/7 for you & registered family within the CHAMP Plan app
  • $0 cost for unlimited Virtual Mental Health Care for you and registered family
  • $0 cost for unlimited Virtual Veterinary Consultations for your household pets
  • $0 copay for Wellness Coaches and individualized content cultivated specifically for you
  • $0 copay for 100% Preventative Care for you

How the CHAMP Plan works!

  • The CHAMP PlanT™ premium is a pre-tax deduction, which is listed on the example attached as “Champion Health.” This deduction lowers your taxable income, meaning you are keeping more of your own dollars in your check
  • Within the same check, individuals will receive a claim payment, seen below as “CHAMP Benefit.” The claim payment helps to offset the pre- tax premium and reduces the cost of the plan for you
  • Champion Health handles ALL the details, while YOU save, and receive benefits, including 24/7 $0 copay virtual direct primary care with CHAMP Virtual Care and paid visits by The CHAMP Plan for primary and urgent care visits

The out-of-pocket cost in this example: $10.32 per pay period or $20.64 per month.

  • Download “The CHAMP Plan” app in your phone’s app store
  • Tap “Get Started” and follow the prompts to get connected to the plan provided by your employer
  • Create your account by entering your name, zip code, email, and creating a password

  • You will be automatically enrolled if you have a positive increase in your net take-home pay
  • Call in to see what your impact from the plan will be and to enroll yourself today. Dependents can also participate. Register them in the app so they can use the CHAMP Plan benefits

  2. Click “Get Started” on the home screen
  3. After completion, you will be notified via email when your results are ready to review
  4. Review your HRA results online or in the App. Refer back monthly to app.champplan.com or The CHAMP App to complete your monthly custom health activities!
